Patient Information
First Visit
Prosthodontists are often considered the architects of the dental team because of the comprehensive approach taken in diagnosing and treating your dental problems. This is why they are often called upon, either by patients or other dentists, for second opinions as to the course of treatment that is necessary to achieve the patient’s desired goal. It is a Prosthodontists responsibility to develop the treatment plan that will address your aesthetic and functional concerns. To this end, Dr. Halmos will spend a great deal of time with you on your initial visit obtaining all of the information necessary to develop his treatment recommendations. Many times, your treatment options will be given to you the same day and treatment will begin soon thereafter. Occasionally, however, a complex medical history or treatment plan will require a more thorough evaluation and a second appointment will be made to discuss your treatment options. This second appointment is at no cost to you.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip (if you are coming to us by referral from another dentist) and any X-rays if applicable
- A list of medications you are presently taking
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.
